Overview
This is the consent form of the Prince Albert Multicultural Council (PAMC) and the YESS program funded by the Government of Canada’s Youth Employment and Skills Strategy.
By signing this consent form, you are agreeing to let PAMC and the YESS program interview, photograph and record you [or the participant] and use that content in its communications for free. You are also agreeing to all other terms explained in this form.
It is your choice to participate. Choosing not to participate will in no way impact your current or future dealings with PAMC, the YESS program or the Government of Canada. If you sign this form and later change your mind, you can withdraw your consent at any time by contacting PAMC.
A parent or legal guardian must complete and sign this form if the participant is a minor.
Privacy Notice
The PAMC and the YESS program of the Government of Canada are committed to protecting the privacy rights and personal information of individuals. Please note that:

any images and recordings (written, audio or video) of you are collected under the authority of the Department of Employment and Social Development Act and will be described in personal information bank Public Communications (PSU 914) or Outreach Activities (PSU 938);

collected images, recordings (written, audio or video) and information may be shared with other federal, provincial, and territorial government departments and trusted non-governmental partners for the purpose of promoting Government of Canada programs and services;

the Government of Canada's terms and conditions for web and social media apply to information shared and interacted with on those mediums;

the collection and use of your personal information for ESDC communications purposes is subject to the Privacy Act, under which you may access your personal information and request corrections; and
